Can i ask if this is SCAM?! Help!

I'm Looking for a job, and i just saw this Job Advertisement, then i emailed them my resume after that i received a response. I don't know if this is true or scam, although they didn't asked me for money, just some papers with my signature on it. So i just wanted to verify if this is a scam or not. Do i have to mail them the paper they asking or not?? Here's what it says:

this is probably a scam because if you read it it says two different things. it keeps saying care givers get $12. then in another paragraph it says care givers are given only minimun wage which I am not sure is &12.
