Photographs :
One passport sized colour photograph. The photograph must:
* Have a white background
* Be taken within the last six months

Proof of Residency Address
Evidence of residency of current address stated on the application form.
* A copy of the applicant's utility bill or tenancy agreement

Application Form

One fully completed and signed application form

It is written by a West African who claims to be USA-based.
It is sent from a domain registered by hidden.
Therefore, it is a scam.

Please post the headers of the email. The website of the domain is suspended, but this registrar happily disables domains completely when we give them enough evidence.

Another target of this scam has sent me headers and the domain has been reported for fraud. The attachments sent by the scammer contain the following address and phone numbers:
1002 5th Avenue, Seattle, WA 98101 (parts of the address don't match)
+1-206-400-2832 (in Washington near Seattle)
+1-650-434-1687 (in California)
