This is an email from the fake agency.

The UK Tax Clearance and Medical Clearance are inventions of the scammer.

An au pair does not usually need a National Insurance Number, but if one is needed, it is obtained by filling in a form and presenting ID at a local Jobcentre in the UK, after the au pair has arrived there. There is no fee to pay for this.
