TIME TO ENROLL!
Dear Valued Employee,
We are excited to announce the continuation of the CBR's S125 Flex Plan to =
include the following:
* CREDIT/DEBIT CARD issued to all participating employees. (EFLEX w=
ill request receipts) Keep debit card if currently participating as it will=
be re-loaded for 2017.
* Immediate online access to view your personal account (funds, cla=
ims etc...)
When you participate in our S125 plan, you can reduce your taxable income a=
nd increase your take-home pay. The money that is deposited into your S125=
account comes straight out of your gross pay, before taxes are calculated.
The following are covered under S125 and eligible for pre-tax savings:
* Group Health Insurance Premiums (automatic upon enrollment)
* Health Care Flexible Spending Account - You MUST be enrolled in a=
medical plan to be able to participate. Up to $2,500 reimbursement for hea=
lth care expenses for yourself and your family which are not covered by hea=
lth insurance; including co-pays, deductibles, dental, vision, orthodontia,=
etc. with TAX-FREE dollars. (Limited for employees currently participating=
in a Health Savings Account (HSA) plan)-Enrollment required every year. E=
mployees may roll over $500 to the following year.
* Dependent Care Flexible Spending Account - reimbursement for elig=
ible child day care or dependent care expenses up to $5,000 per year TAX-FR=
EE. Enrollment required every year.
If you would like to participate in or continue your enrollment in the S125=
plan for 2017, please complete the attached enrollment form and email to M=
aria Silversmith at [email protected]<mailto:[email protected]> or=
fax to CBR at 888-294-8763; Attention Maria.
Please remember that this is the ONLY opportunity to enroll in the S125 pla=
n for January 1st, 2017. The deadline for submitting your enrollment form t=
o CBR for the 2017 plan year is, November 30th, 2016.
Please note that this is a use it or lose it benefit. If you do not use all=
of your funds for the 2017 calendar year only $500 will rollover to the 20=
18 benefit year. If you are terminated or voluntarily quit anytime during t=
he 2017 calendar year, you will lose all available funds.
IRS regulations require nondiscrimination testing under Code 105(h), 125, a=
nd 129 to insure that employees are treated fairly in the design and functi=
on of the plan. Participating employees shall not include self-employed i=
ndividuals as provided in Code Section 401(c)(2) such as sole proprietors, =
outside directors of a corporation, partners or more than 2% shareholders o=
f an S Corporation. CBR will be conducting the nondiscrimination testing.

TIME TO ENROLL!
Dear Valued Employee,
We are excited to announce the continuation of the CBR's S125 Flex Plan to =
include the following:
* CREDIT/DEBIT CARD issued to all participating employees. (EFLEX w=
ill request receipts) Keep debit card if currently participating as it will=
be re-loaded for 2017.
* Immediate online access to view your personal account (funds, cla=
ims etc...)
When you participate in our S125 plan, you can reduce your taxable income a=
nd increase your take-home pay. The money that is deposited into your S125=
account comes straight out of your gross pay, before taxes are calculated.
The following are covered under S125 and eligible for pre-tax savings:
* Group Health Insurance Premiums (automatic upon enrollment)
* Health Care Flexible Spending Account - You MUST be enrolled in a=
medical plan to be able to participate. Up to $2,500 reimbursement for hea=
lth care expenses for yourself and your family which are not covered by hea=
lth insurance; including co-pays, deductibles, dental, vision, orthodontia,=
etc. with TAX-FREE dollars. (Limited for employees currently participating=
in a Health Savings Account (HSA) plan)-Enrollment required every year. E=
mployees may roll over $500 to the following year.
* Dependent Care Flexible Spending Account - reimbursement for elig=
ible child day care or dependent care expenses up to $5,000 per year TAX-FR=
EE. Enrollment required every year.
If you would like to participate in or continue your enrollment in the S125=
plan for 2017, please complete the attached enrollment form and email to M=

fax to CBR at 888-294-8763; Attention Maria.
Please remember that this is the ONLY opportunity to enroll in the S125 pla=
n for January 1st, 2017. The deadline for submitting your enrollment form t=
o CBR for the 2017 plan year is, November 30th, 2016.
Please note that this is a use it or lose it benefit. If you do not use all=
of your funds for the 2017 calendar year only $500 will rollover to the 20=
18 benefit year. If you are terminated or voluntarily quit anytime during t=
he 2017 calendar year, you will lose all available funds.
IRS regulations require nondiscrimination testing under Code 105(h), 125, a=
nd 129 to insure that employees are treated fairly in the design and functi=
on of the plan. Participating employees shall not include self-employed i=
ndividuals as provided in Code Section 401(c)(2) such as sole proprietors, =
outside directors of a corporation, partners or more than 2% shareholders o=
f an S Corporation. CBR will be conducting the nondiscrimination testing.
